1. Initial Environmental Examination (IEE) Full Blown CF IEE Checklist Form - CBFM

• Location Map plotted on the Topographic map with 1:50,000 scale
• Project site/perimeter map verified by the CENRO (Certification from CENRO)
• Resource/timber inventory (10%)
• Present forest condition (land resource condition) map

2. Status of Land / Land Claim

• Certification/evaluation from the CENRO or PENRO
• Land title/tax declaration/stewardship contract whichever is applicable
• For conflict areas: Waiver from other claimants

3. Endorsement from the Local Government Units with attached resolution including a copy of the documentation of the consultation process

• Sangguniang Pambarangay
• Sangguniang Bayan
• Indigenous tribal communities: In case the area falls within an ancestral land/domain

4. Resource Management Plan / Community Resource Management Development Plan (CRDMP)

• Proposed Development/Management Plan and Annual Operations Plan
• Census and Description of dependent communities/areas occupants

5. For application for renewal, evaluation of past performance of the permitee by the CENRO

6. Inspection Chart Maps (for FLMA, per DAO No. 71 S. 1990)
