2. Endorsement (with resolution and documentation of the consultation process) from the following:

1. )  Sangguniang Pambarangay
2. )  Sangguniang Bayan/Panglungsod
3. )  Indigenous Cultural Communities (ICC's)

3. For research projects covered by DENR DAO 96-37:

1.   Initial Environmental Examination (IEE) document (refer to DENR DAO 96-37,
     Series of 1996, for contents of IEE)
2. )  Location map/project site plotted on the topographic map with 1:50,000 scale
3. )  Map of the study area noting adjacent water bodies (scale 1:10,000)

4. For research projects dealing with prospecting of biological and genetic resources (Executive Order No. 247):

1. )  Letter of acceptance from Filipino counterparts authorized by or representing the host institutions


2. )  Letter of endorsement from the head of institution where applicant is affiliated


3. )  Company/Institution/Organization/Agency Profile


4. )  Prior Informed Consent obtained in accordance with section 7 of E.O. 247 from the following:



1. Indigenous Cultural Communities/Indigenous Peoples (IPS) - in cases where the prospecting of biological and genetic will be undertaken within their ancestral domains/lands;



2. Local Communities (LC) - in cases where the prospecting of biological and genetic resources will be undertaken within their area/s of jurisdiction.



3. Protected Area Management Board (PAMB) - in cases where the prospecting of biological and genetic resources will be undertaken within a protected area. Provided that, if the PAMB for a certain protected area has not been organized, a letter of consent shall be obtained from the concerned Regional Executive Director under whose jurisdiction the protected area is located;



4. Private Land Owner - in cases where the prospecting of biological and genetic resources will be undertaken within the private land.
