a. Water Supply Systems

EIS: more than six production wells and other systems (e.g. infiltration gallery,)
IEE: six or less production wells

b. Water Supply System (Distribution Only) IEE: Level III

c. Irrigation System (distribution system only)

EIS: >=1,000 hectares service area
IEE: < 1,000 hectares service area
CNC: < 300 hectares

d. Impounding System or Flood Control

EIS: Reservoir (flooded area) > = 25 hectares
IEE: Reservoir (flooded area) , 25 hectares

e. Pipelines

1. Fuel Pipelines

EIS: Length >=25 kilometers
IEE: Length < 25 kilometers

2. Other Pipelines

EIS: Length >= 50 km
IEE: Length < 50 km

Additional requirements to be attached to the EIS:

• Location map/Project site plotted on the Topo Map with 1:50,000 scale
• Site Development Plan/Map Noting adjacent water bodies (scale: 1:10,000)

2. Endorsement from the following LGU with resolution including a copy of the documentation of the consultation process undertaken.

• Sangguniang Pambarangay
• Sangguniang Bayan/Panlungsod
• Sangguniang Panlalawigan
• Free and Prior Informed Consent of IPs in case the area falls within a certified ancestral land/domain.

3. Zoning Clearance from the Municipality/City Zoning Office

4. Proof of land ownership, land claim or land occupancy

• Land Title/Tax Declaration/Stewardship Contract/Other Instruments
• Certification/evaluation from CENRO/PENRO
