RESOURCE EXTRACTIVE ACTIVITIES

1. Major Mining and Quarrying Activities

   a. Mineral Ore processing

   EIS: >=70,000 MT annual processing capacity (inputs)
   IEE: < 70,000 MT annual processing capacity per day extraction rate

   b. Cement, other cement products, clinker, limestone and other non-metallic    minerals processing plant

   EIS: >=75,000 MT per annum or quarry area >=20 hectares
   IEE: < 75,000 MT per annum and quarry area < 20 hectares

   c. Off-shore mining (including extraction of deuterium, oil and gas)

   EIS: Regardless of capacity or area

2. Additional requirements to be attached to the EIS:

• Geo-hazard map of the applied area
• Location map/project site plotted on the Topo map with 1:50,000 scale
• Site development plan/map noting adjacent water bodies (scale : 1: 10,000 scale)
• Two-year Exploration Work Program
• Proposed Commercial Production Work Program/Construction and Development Work Program, whichever is applicable

3. Status of Land/Land Claim

• Certification from CENRO/PENRO
• Land Title/Tax Declaration/Stewardship Contract
• For conflict areas: waiver from other claimants

4. Endorsement from the following LGU with resolution including a copy of        documentation of the consultation process

• Sangguniang Pambarangay
• Sangguniang Bayan/Lungsod
• Sangguniang Panlalawigan
• Indigenous tribal communities in case the area falls within a certified ancestral land/domain

5. Zoning Clearance from the Municipal/City Zoning Office

  In case of renewal or expansion, environmental accounting of previous operation.
